Site information and data sheet templates are available below in both .xls and .xlsx formats. 

Use these templates when submitting site information and when collecting data. 

Site information only needs to be submitted once, data sheets should be submitted annually.

Email your completed site information and data sheets to the DRAGNet coordinator.
